Which instruction would the nurse give the client who has a new prescription for hydrochlorothiazide?Take hydrochlorothiazide exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor. Hydrochlorothiazide controls high blood pressure but does not cure it. Continue to take hydrochlorothiazide even if you feel well.
Which of the following adverse effects should the nurse monitor for in a patient taking warfarin?Since warfarin is an anticoagulant, monitoring for signs and symptoms of bleeding such as black tarry stools, nosebleeds, or hematomas is imperative.
What should the nurse monitor in a client who is taking blood pressure medication?Monitor BP and pulse rate. Dosage is usually adjusted to patient's blood pressure because it can cause hypotension, bradycardia, and sedation. Rebound hypertension may occur if stopped abruptly. Patients should be compliant with medication therapy and take the medication at the same time each day.
Where does the nurse store the reconstituted form of this penicillin? After Reconstitution: Store in a refrigerator and use within 48 hours. If kept at room temperature, use within 24 hours. See package insert to complete information. Each vial contains 1000 mg of medication.
